A translational approach to design effective intervention tools for informal caregivers of dependent cancer patients.
2019
OBJECTIVES: Caregivers of terminal patients often report a higher prevalence of unmet needs than cancer survivors. However, very few interventions have been carried out to support caregivers of patients in advanced stages, and, in most cases, they have not been rigorously designed and evaluated. The ultimate aim of this research was to obtain specific information about the sociodemographic characteristics, the different types of care provided, the symptoms due to burdens, the impact of caring on the quality of life, and the unmet needs of informal caregivers of dependent patients with cancer. This is to design effective intervention programs that can be implemented from the hospital setting…
Psychosocial and occupational risk perception among health care workers: a Moroccan multicenter study.
2015
Background International studies on occupational risks in public hospitals are infrequent and only few researchers have focused on psychosocial stress in Moroccan Health Care Workers (HCWs). The aim of this study was to present and analyze Moroccan HCWs occupational risk perception. Across nine public hospitals from three Moroccan regions (northern, central and southern), a 49 item French questionnaire with 4 occupational risks subscales, was distributed to 4746 HCWs. This questionnaire was based on the Job Content Questionnaire. Psychosocial job demand, job decision latitude and social support scores analysis were used to isolate high strain jobs. Occupational risks and high strain percept…
Management of ulcerative colitis in a real-life setting: An Italian multicenter, prospective, observational AIGO study
2019
Abstract Background No data are available on the variability in the clinical management of ulcerative colitis (UC) patients by Italian gastroenterologists. Therefore, improving the standards of UC care as provided by the National Welfare Clinical Path (PDTA), in accordance with the European Crohn’s and Colitis Organization (ECCO) guidelines for UC, is not easy. Aims To assess the management of UC by Italian gastroenterologists in a real-life setting taking into account its variability. Methods This prospective, cross-sectional, observational study included IBD-specialized gastroenterologists (GSIBDs) and general gastroenterologists (GGs) working in Italian public hospital units. Consecutive…
Cervical Cancer Screening in Women With Severe Mental Disorders An Approach to the Spanish Context
2018
Background The incidence of invasive cervical cancer and its mortality have been reduced through primary and secondary prevention. Screening rates tend to be lower in vulnerable groups, such as people with severe mental disorders, who have a later detection of cancer and a higher mortality. The access of these women to cervical cancer screening is uncertain in our context. Objective The aim of this study was to determine the cervical cancer screening rates in women with severe mental disorders. Methods This was a descriptive cross-sectional study. Women 25 to 65 years old who were admitted during 2016 to the psychiatric unit of a public hospital in Spain were included in the study, and it w…
Overcoming traditional Performance Management in Public Healthcare Institutions: The need for a DPM Approach
Donabedian (1988) was the scientific father of the notion that Performance Measurement (PM) of hospital outputs leads to improved quality of healthcare services, setting the foundations of how hospital quality is still today defined, considered and measured using three different kinds of measures: structure, process and outcome measures. Nonetheless, a growing part of literature during the last two decades has started doubting those favourable outcomes of PM on hospital quality (De Vos, et al., 2009; Nolan & Berwick, 2006; Werner et al., 2008a, 2008b; Werner & Bradlow, 2006; Wright & Hershman, 2014) while other research shows that ineffective PM systems are not just useless but …
Ecological momentary assessment for chronic pain in fibromyalgia using a smartphone: A randomized crossover study
2013
Background Daily diaries are a useful way of measuring fluctuations in pain-related symptoms. However, traditional diaries do not assure the gathering of data in real time, not solving the problem of retrospective assessment. Ecological momentary assessment (EMA) by means of electronic diaries helps to improve repeated assessment. However, it is important to test its feasibility in specific populations in order to reach a wider number of people who could benefit from these procedures. Methods The present study compares the compliance and acceptability of an electronic diary running on a smartphone using a crossover design for a sample with a specific pain condition, fibromyalgia and low fam…
Carpal tunnel release surgery: small-area variation and impact of ambulatory surgery in the autonomous region of Valencia, Spain
2013
Objective: This study aimed to analyze variability in rates of carpal tunnel release surgery among the healthcare areas of the autonomous region of Valencia, and to evaluate the contribution of ambulatory surgery and referrals to private hospitals to the variability found. Methods: We carried out a cross-sectional, population-based study, describing the rates of carpal tunnel release surgery, standardized by age and sex, among areas in the region of Valencia in 2006. The observed variation was then analyzed using small-area analysis methods. Data from hospital admissions, referrals to private hospitals, population statistics and hospital resources were used to construct standardized rates, …
THE PUBLIC HOSPITAL AS A SYSTEM: FINANCING POLICIES AND THEIR LONG-TERM IMPACT ON THE HEALTH SERVICES PROVIDED
Prescribing behaviour of practitioners in public and private hospitals in Pakistan evaluated using the World Health Organization (WHO) indicators: A …
2017
Summary Introduction The non-rational use of medicines by medical practitioner in their prescribing practices might lead to therapeutic failure. A critical perspective on Integrated Project Delivery (IPD) applied in a Norwegian public hospital project
2020
Does the IPD concept deliver as required and expected, and if not, how can that be explained? This paper is a critical realist inspired methodology based on a combination of the inductive and deductive approaches used in case study research. IPD is based on relational contracting between multiple parties, in this case between the Owner, Contractor, MEP subcontractors and a group of Consulting engineers who share control of the project. At the core of the concept is shared risk and opportunities among the parties in the IPD contract. Our theoretical perspectives are based mainly on the Principal-Agent theory (PA), Transaction Cost theory (TC), and its related incentives. This paper reports o…
